There is a possibility that a cable that runs near the house battery inverter terminal inside the unit may chaff against that terminal. The results can be seen in my photo album. Beware!
 
More information about the issue you encountered would be helpful to others with this installation. The photo is frightening but doesn't tell much of a story. Electrical issues are to common in boats and can be dangerous. It is usually not a flaw in design but a simple installation issue that can cause failure like you experienced. Improper wiring, fuse sizing. a simple tie strap removed or never installed, simple mistakes. Information like this can help prevent this from happening to someone else. Fortunately the safety features installed prevented this from being more sever.
 
BB, as you noted this could be a one in a million issue. There was no warning of a failure. When I plugged into shore power the sparks flew! The way the inverter is mounted against the hull probably allowed the multi conductor cable to rest against the bare metal terminal. Time and vibration did the rest. Again, as you noted, a simple zip tie would fix it.
