inverter failure on R25 2008

We recently cruised to Anacortes, and hooked up to shore power. While away from the boat, the cabin filled with smoke
from a fried inverter. What would have caused this to occur? Also, could this have created more trouble for the electrical system?
 
Curious... was the Inverter active (i.e., turned on). On my R-25 to use the Xantrex 1800 watt inverter I have to... (my notes)

Using the Inverter when ‘on the water’

1. Ensure Inverter main switch is ON (on our R-25 the Inverter is under port side step and up against hull side in cockpit)
2. On the Inverter panel next to radio set the switch ON
3. On AC Distribution panel to right of helm turn off the Shore power breaker and then move slider to block the SHORE position and flip the GENERATOR breaker switch ON.
